Per the Northwest Line Agreement, the Electrical Workers' HRA began contributions per hour in 2008. The plan sent out enrollment application materials to each participant who now has an account with the plan. IT IS IMPORTANT THAT THE ELECTRICAL WORKERS' HRA RECEIVE A SIGNED ENROLLMENT FORM FROM EACH PARTICIPANT. Participants who fail to submit an enrollment form will forfeit their account balance to the Plan to pay operating expenses if a signed enrollment form is not returned within: 1) six months of the date of the last contribution if their account balance is less than $250; or 2) twenty months of the date of the last contributions if their account balance is more than $250.
Please complete and return your paperwork. If you cannot locate your paperwork please contact the Third-party Administrator (TPA), REHN and Associates, PO Box 5433, Spokane, Washington, 99205-0433, 1-800 VEBA101 (832-2101). Please contact the Local Union 659 if we can be of further assistance.
Our office has reviewed the list provided by the HRA and have posted upon the "Member Resources" site those participants who have failed to provide enrollment application materials that are Local Union 659 members. Please request a login if you are a Local Union 659 member and from this list if you see a name you recognize tell your Brother to complete the application materials now!
